Burial Transit Permit (Repatriation of Human Remains)

The Federal Ministry of Health in Nigeria requires a waiver before human remains may be repatriated to Nigeria. A request must be sent to the Honourable Minister of Health in Abuja, and supporting documents must also be submitted to the High Commission.

Requirements

  • Formal Request Letter – to the Honourable Minister, Federal Ministry of Health, Abuja (include date/place/cause of death and age).
  • Burial Transit Permit.
  • Certification of Embalming & Preparation of Human Remains.
  • Notarized Statement of Funeral Director (confirming casket contains only the remains).
  • Medical Examiner’s Report (if applicable).
  • Certificate for shipment of body outside Canada.
  • Medical Certificate of Death.
  • Statement of Death.
  • Notarized copy of Death Certificate.
  • Certification of Vital Record (regarding communicable/non-communicable diseases).
  • Fees – CAD $50 per page plus CAD $50 administrative fee (money order only).
  • International Waiver Fee – ₦5,000 payable to Port Health Services in Nigeria.
  • Original Nigerian Passport of the deceased (or notarized letter from relative if unavailable).
  • Return Envelope – self-addressed Canada Post Priority/Xpresspost envelope (National envelope if outside Ontario/Quebec).
  • Original and photocopies of all documents, mailed to the High Commission.

10 business days.
